Care Partners' mission is to provide hospice and palliative care services. The intern will assist the Quality and Performance Improvement Manager to identify and prioritize opportunities for performance improvement utilizing clinical outcome data in the Strategic Health Partners (SHP) quality management database. The intern will assist the Director of Quality in "mining data" from the electronic medical record to measure the outcomes of performance improvement initiatives with a focus on improving patient pain and symptom control at end-of-life. The intern will access the online SHP database, export data into Excel spreadsheets, prepare graphs and develop control charts. The intern will present data to interdisciplinary groups of health care professionals, who will generate interventions to improve clinical outcomes for patients. The intern will then collect data from the electronic medical record to evaluate intervention impacts and report back to the intervention teams. While the intern will not work directly with patients, this position requires a high level of comfort relating to palliative and end-of-life care, patients’ rights, and the dying process.
